Social Services: With detailed case management, we address each individual’s unique needs, guiding them through their life's journey with tailored care coordination and transition services designed for empowerment.

Medicare Counseling: Our Medicare Counseling demystifies health care options, presenting clear, personalized advisement for everyone.

PEARLS Counseling: Through PEARLS, we delve into the mental and emotional realms, encouraging active, rewarding senior lifestyles.                                             

VITA Tax Assistance: Our VITA Tax services extend a helping hand during tax season, ensuring precision and understanding for each tax preparation need.

Evidence-Based Health Education: Accessible, grounded health education empowers informed decision-making for robust living.

Supplemental Needs Boxes: Our carefully assembled Supplemental Needs Boxes, which include necessities from meal replacements to toiletries, preserve daily dignity.

Telephone Reassurance: Our Telephone Reassurance offers a dependable, comforting voice at the other end of the line, ensuring no one feels alone.

Durable Medical Equipment: Assisting mobility and independence, our Durable Medical Equipment helps transform day-to-day life.

My Ride Connection: Responding to the vital importance of accessible transportation.  It ensures that our community members retain autonomy and connectedness, linking them with the places and people that enrich their lives
